Formstack started as a way for me to easily build lead gen forms on our website. Today, nearly every single department uses it nearly every single day to capture data.

I’ve used many survey tools but I always go back to Formstack because it’s the only tool that allows me to build a form, get results, and report on it in a timely fashion. I’ve found that other tools are much more cumbersome and slow.

Formstack’s digital COVID-19 self-declaration form allowed us to quickly and securely screen employees each day before they entered the workplace. It has been a vital part of our operations as we continue to navigate this new normal.
